The HOPE team provides MHSA FSP. This includes services to adults with serious mental health conditions or co-occurring mental health and substance use disorders. Participants in the HOPE program receive intensive support towards recovery goals and are encouraged to fully participate in the Wellness and Recovery Center (SYBH). The goal of this program is to help participants reach and maintain stability, participate fully in community life, decrease isolation, increase independence and support a sense of belonging. Services are provided based upon participants individual wellness and recovery goals.
Services can include intensive case management and rehabilitation services, living arrangement assistance, facility placement, individual and group services, collateral and peer support programs, and access to employment and housing resource specialist.
Intervention counselors are available to clients on a 24/7 basis.
In addition, The HOPE team provides elder adult services. This includes behavioral health services and episodic treatment services to adults aged 60 and older. The treatment services are provided to elder adults residing in both Yuba County and Sutter County, who are underserved, unserved, and physically or geographically isolated. The program is designed to provide services to reduce negative psychiatric symptoms through psychotherapy and assist clients in achieving personal goals. The vision of the this program is to provide services that are appropriate and that make clients feel empowered and listened to at their level of independence in all settings. The vision is also to provide advocacy and ensure services are client-driven and client-friendly.
The program therapist works in collaboration with individuals, their families, social services agencies, public health, the medical community, senior centers, law enforcement, and senior housing entities. Alongside outreach, the therapists provide assessment, individual therapy, case management, linkage to the resources and supports needed, medication support and substance use disorder treatment.
The therapists are available to provide community presentations on older adult mental health issues, how to access care and linkage to services. Services can be assessed by attending the Adult Services Open Access Clinic.

Monday - Friday 8 am to 5 pm
For New Clients/Intakes:
Monday - Wednesday 8 am to 2 pm (Walk-In, Telephone, Tele-Health Video) Appointments can be requested for those in care facilities or who cannot come in for a triage.
