The Path Program seeks to provide services to persons experiencing homelessness who are difficult to engage in services or serve in traditional shelters. The program consists of housing specialist, resource specialist and case managers who provide triage, screenings and assessments to link client to appropriate services. The Path program seeks to connect clients to behavioral health, and substance abuse services as needed. Along with linkage to supportive housing resources with education, trainings, and support that seeks to help clients maintain housing as appropriate with the help of case management services. Transportation to services and providers can be provided with the program to help link clients to services.
