The iCARE mobile engagement team serves individuals who are living with untreated severe or chronic behavioral health conditions who experience high utilization of emergency or inpatient care, and/or who have high levels of contact with law enforcement but have remained unengaged with ongoing care for their condition. The iCARE team is focused on getting to know clients, understanding their ideas about personal wellness, desires for their own life, building trust and spending time getting to know client needs. The iCARE mobile engagement team is not a crisis team or a case management team, but works closely with SYBH’s crisis, case management and FSP teams. The iCARE engagement team will link clients when they are ready, with outpatient treatment and support resources, accompanying clients to treatment services as needed and upon client request.
