Home MenuCommunity Health Improvement Plan
In 2022, Sutter County Public Health completed the Sutter County Community Health Assessment (CHA). This assessment provided insight on illnesses affecting community members, how they experience illness, and factors that influence individual health and well-being. During the CHA process, three health priority areas were identified that provided a foundation for the Community Health Improvement Plan (CHIP):
- Addressing Homelessness
- Building Resilient Communities (BRC)
- Reducing Sexually Transmitted Infections (STIs)
The CHIP is the guiding document for the next 3 to 5 years to address these health priority areas. Implementing the CHIP in our community is a critical first step in paving the way towards building a healthier Sutter County. The power of this plan lies in utilizing our unique community strengths, building partnerships, and developing ownership of the health of our community to transform the health and well-being for all residents. The CHIP is a living document, hence, we will continue to update this document as we learn and discuss new needs and strategies as a community.
